This portal will provide you with tools to communicate with your doctor and manage your health records.

• Enter or modify personal information
• View your medical history, medications and test results
• Request a prescription refill
• Communicate with our staff
• Request medical records
• Submit forms and agreements online
• Pay your bill and receive online patient statements

EXISTING PATIENTS
Our secure, HIPAA-compliant patient portal is automatically activated for any patient with an appointment scheduled and an email address on file. An email should have been sent with your temporary password to login. Your username will always be your email address. If you have not provided our office with an email address and wish to have access to your portal account, please contact our office.  

DO NOT USE THE PORTAL FOR ANY URGENT MEDICAL PROBLEMS.
